29 CFR 1926.451(c)(2)(iv):     Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use.      a.  At the site, an employee was operating a forklift in a manner that was not designed by the manufacturer.  The forklift was used to elevate personnel in a work platform at a height of 13 feet, thereby exposing the employees to caught-between and fall hazards.

29 CFR 1926.451(f)(5):     Scaffolds shall not be moved horizontally while employees are on them, unless they have been designed by a registered professional engineer specifically for such movement or, for mobile scaffolds, where the provisions of 1926.452(w) are followed.        a)  At the site, employees gained access from the ground to the roof by riding in a work platform that was attached to a forklift.  The forklift operator would travel in forward and reverse in a horizontal motion while simultaneously moving the employees in a vertical position, thereby exposing employees to caught-between and fall hazards at a height of approximately 13 feet.

29 CFR 1926.501(b)(10):     "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section,each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall beprotected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrailsystem, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safetymonitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring systemalone [i.e. without the warning line system] is permitted.    a)  At the workplace, there were employees working on a low-slope roof without being connected to the anchor or horizontal lifeline of a fall arrest system when outside of the warning lines, thereby exposing the employees to a fall hazard of approximately 13 feet.
