29 CFR 1910.145(c)(2)(i):     Caution signs were not used to warn against potential hazards or to caution against unsafe practices:    On March 21, 2012, Eastland Feed and Grain, Inc. did not post caution sings warning employees of struck-by hazards associated with the operation of overhead commercial doors that were not equipped with safety devices.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.146(c)(2):     The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.    On or about March 15, 2012, Eastland Feed and Grain, Inc. employees entered dump pit boots that were not designated as a permit required confined space.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.146(e)(1):     Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit:  Note:  Appendix D to 29 CFR 1910.146 presents examples of permits whose elements are considered to comply with these requirements.    On or about March 15, 2012, Eastland Feed and Grain, Inc. employees entered dump pit boots without completion of an entry permit that would document measures had be taken such as, but not limited to, testing the atmosphere to verify that a safe entry could be made into the work area.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.272(j)(1):     The employer did not develop and implement a written housekeeping program that established the frequency and the method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other exposed surfaces.    On March 21, 2012, Eastland Feed and Grain, Inc. permitted grain dust accumulation of in excess of .25 inches on the upper interior horizontal surfaces in the grinding and mixing areas.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
