29 CFR 1926.251(a)(1): Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service.    A.  Gleason Construction Co. Inc. worksite located near 221 Pine St., Perrysburg, Ohio: On or about March 19, 2012, the employer did not ensure the hook being used to hoist steel plates had an operable safety latch. The hook was being used to hoist 20 foot long steel plates when it became unhooked resulting in the plate being suspended in the trench and not secured. Inspection of the hook revealed that the tip of the hook was bent beyond the plane of the hook.    per ISA AVD changed to: A.	Gleason Construction Co. Inc worksite located near 221 Pine   St., Perrysburg, Ohio:  On or about March 19, 2012, the employer  did not assure the hook being used to hoist steel plates was not bent.  The hook was being used to hoist 20 foot long steel plates when it become unhooked resulting in the plate being suspended in the trench and not secured.  Inspection of the hook revealed that the tip of the hook was bent beyond the plane of the hook.      Gleason Construction Co. Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.251, which was contained in OSHA inspection number 309449643, citation number 1, item number 1 and was affirmed as a final order on February 8, 2012, with respect to a workplace located at Perrysburg, OH.
