29 CFR 1910.147(c)(1):  The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a) Leadar Roll Inc. located on 893 Shawnee Road in Lima, OH:  On or about March 22, 2012, the employer did not establish an energy control program for equipment in their facility such as, but not limited to the Crawford-Swift lathe.  The maintenance tech was exposed to uncontrolled electrical energy sources while servicing the machine.

29 CFR 1910.333(a):  Safety-related work practices shall be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards:       a) Leadar Roll Inc located on 893 Shawnee Road in Lima, OH:  On or about March 22, 2012, the employer did not ensure that maintenance personnel working on 480 volts, 3-phase energized control circuits followed safe work practices.  Maintenance personnel were not aware of safe approach distances, and were not aware of the need for electrical personal protective equipment when servicing equipment such as but not limited to the Crawford-Smith lathe.  Maintenance personnel checks fuses and voltage with a Fluke 23 series III multimeter.
