29 CFR 1910.132(d)(1)(iii): The employer did not select personal protective equipment (PPE) that properly fits each affected employee:       (a)	Akal Security, Inc. at 1823 Stout Street, Denver, CO 80294: On and before 4/2/12, the employer did not select personal protective equipment (PPE) that properly fits each affected employee. Employees are provided with expired and improperly fitted body armor. This condition prevented the body armor from serving its purpose to protect from bodily harm due to firearms.      Abatement Note: The employer shall ensure that workplace hazard determinations include, at least, the following steps:                     1) PPE selection and that each affected employee uses the types of PPE that will protect them from the hazards identified in the hazard assessment;   	2) Communicate selection decisions to each affected employee; and  	3) Ensure that PPE properly fits each affected employee.       Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
