29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): a) On or about February 16, 2012, the employer, Four:S Contracting, did not ensure employees, who were required to repair sewer pipes in an approximately 13 foot deep trench of class C soil, were protected from cave-ins by an adequate protective system thereby exposing employees to crushing hazards. FOUR:S CONTRACTING WAS PREVIOUSLY CITED FOR A VIOLATION OF OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.652(a)(1) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION 310466818, CITATION 1, ITEM 1B, ISSUED JUNE 12, 2007, WITH RESPECT TO THE WORKPLACE LOCATED AT 319 S. HAZEL COURT, DENVER, COLORADO 80219. THE CITATION BECAME FINAL ON MAY 7, 2008. THE FINAL ABATEMENT DATE FOR THIS CITATION WAS JULY 11, 2007.
