29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  (a) On or about March 22, 2012, and at times prior, Aztec General Contractors, LLC, as controlling employer, did not ensure that employees of J & D Stucco Inc. were protected from fall hazards when they accessed the top three levels of the five-level scaffold and top four levels of the six-level scaffold by climbing the crossbraces. 29 CFR 1926.451(e)(5)(i):  Ramps and walkways 6 feet (1.8 m) or more above lower levels did not have guardrail systems that comply with subpart M of this part -- Fall Protection:  (a) On or about March 22, 2012, and at times prior, Aztec General Contractors, LLC, as controlling employer, did not ensure that employees of J & D Stucco Inc. were protected from fall hazards when they accessed the roof from the scaffold using ramps that were not guarded at a firehouse under construction at 2201 Lake Ave. in Pueblo, Colorado.
