On 5/23/12, one employee working on the pour line was exposed to noise at a dose in excess of the fifty percent dose Action Level.  The employee was exposed to noise at a dose of 108.0%.  The employer did not obtain a new audiogram, at least annually after obtaining the baseline audiogram. The employer required employees to wear 3M tight fitting air purifying half mask respirators with organic vapor cartridges and P100 filters when fabricating molds.  The employer did not provide a medical evaluation to determine an employee's ability to use a respirator.

Work processes involved spreading the mixture by hand within the molds.  Employee's gloves had direct exposure to the mixture containing diphenylmethane-4,4'-diisocyanate.  Employees wore Ansell Blue TNT disposable nitrile medical exam type gloves.  Nitrile disposable medical exam type gloves do not provide adequate protection from direct contact with diphenylmethane-4,4'-diisocyanate.  The employer did not provide appropriate hand protection for direct contact with diphenylmethan-4,4'-diisocyanate.  This condition exposed one employee to a chemical hazard. Employees wore Ansell Blue TNT disposable nitrile medical exam type gloves.  Nitrile disposable medical exam type gloves do not provide adequate protection from direct contact with naphtha.  The employer did not provide appropriate hand protection for direct contact with naphtha.  This condition exposed one employee to a chemical hazard.

Employees fabricated architectural stone using pigment and precast concrete.  Pigment is applied by throwing the powder into the mold.  A pigment, Rockwood Pigments Chromium Oxide Green (containing <500ppm chromic acid and chromates as hexavalent chromium), was used to produce green tinted architectural stone.  The employer did not evaluate employee exposure to hexavalent chromium.  This condition potentially exposed employees to a respiratory hazard.
