29 CFR 1926.451(c)(3): Supported scaffold poles, legs, posts, frames, and uprights shall be plumb and braced to prevent swaying and displacement.      a. 109 State Route 23 South, Riverdale, NJ, North side of building.    Employees were observed working from a tubular framed scaffold that was not cross braced on the interior and exterior sides.  Scaffold was only cross-braced with one brace per vertical section, thereby exposing employees to fall hazards and collapse hazards.          Violation observed on or about 03/23/2012

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, shall be provided to assist employees in mounting and dismounting the ladder.  In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    a. 109 State Route 23 South, Riverdale, NJ, East side of building.      Employees accessed the building roof using a ladder that did extend at least 3 feet above the upper landing surface, thereby exposing employees to fall hazards.       Violation observed on or about 03/23/2012

Employees used a Werner aluminum extension ladder in a manner for which it was not designed.  Employees dismantled the ladder and used each piece as a separate unit, thereby exposing employees to fall hazards.       Violation observed on or about 03/23/2012

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports.       a. 109 State Route 23 South, Riverdale, NJ, North side of building.    Employer did not ensure that the tubular framed scaffold was fully planked.  Scaffold had only a single board on each scaffold section where work was performed, thereby exposing employees to fall hazards.      Multi Construction was previously cited for a violation of this Occupational Safety and Health standard or its equivalent 29 CFR 1926.451(b)(1)(ii) which was contained in OSHA Inspection number 312863822, Citation Number 1, Item Number 2, which became a Final Order on 08/26/2010.
