29 CFR 1926.351(b)(1): All arc welding and cutting cables were not completely insulated, flexible type, capable of handling the maximum current requirements of the work in progress, taking into account the duty cycle under which the arc welder or cutter is working: On or about March 7, 2012, and at times prior thereto, employees were using a Lincoln Electric Ranger 8 welder, Model 10541, S/N K1616-1, in which the cables were not fully insulated where the cables connected to the terminals, due to the absence of terminal boots. This exposes employees to hazards associated with electrical shock.

29 CFR 1926.351(b)(4): Cables in need of repair will not be used. When a cable, other than the cable lead referred to in paragraph (b)(2) of this section, becomes worn to the extent of exposing bare conductors, the portion thus exposed shall be protected by means of rubber and friction tape or other equivalent insulation: On or about March 7, 2012, and times prior thereto, employees were using a Lincoln Electric Ranger 8 welder, S/N K1616-1, in which cable wires were exposed at the area where the cable was connected to the welder clamp. This exposes employees to hazards associated with electrical shock.
