29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years.     (a) Employer did not evaluate all second shift powered industrial truck operators' performance in the past three years while assigning them work duties requiring daily operation of order pickers and propane-gas powered forklifts in the warehouse.    THE LINDSTROM METRIC, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.178(l)(4)(iii) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314038563, CITATION NUMBER 01, ITEM NUMBER 005(b) AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 23, 2010 WITH RESPECT TO A WORKPLACE LOCATED AT 2750 GALVIN DR. ELGIN, IL 60124.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    (a) Waste Management trash compactor C421101 dock at the south-east warehouse docks area. The cable for the safety interlock device on the guard gate for the trash compactor was severed and the key was left in the interlock device. Guard gate was tied in the open position, exposing employees to moving packer ram when dumping trash by hand.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.178(p)(1): Powered industrial truck found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition.      (a) Warehouse and docks area. The operator's seat of the Toyota propane gas powered forklift 12447 was severely damaged and missing cushionings. Employee could suffer back strain or contusions from operating forklift with a damaged seat.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
