29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and shall address at least the elements outlined in (i)(A) through (i)(G): On or about March 13, 2012, and times prior thereto, employees were exposed to fire/explosion hazards in that the employer did not establish and utilize a written procedure to prevent overfilling of condensate tanks, and did not develop and implement an emergency shutdown procedure for filling of the condensate tanks.
