29 CFR 1910.212(a)(5):     Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:    Line Wi Welding area, a six foot tall pedastel fan used by the welders on as needed basis, had a five inch opening at the center. The condtion exposed the employees to amputation and or laceration hazards.

29 CFR 1910.303(b)(1):     Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    a) Near Line WI of the welding area, a metal electrical outlet box used to power the pedestal fan had the conductor wires exposed to damage. The outer insulation sheath was pulled awy from the box. This condtion exposed the employees to electrical hazards.    b) Near Line WI of the welding area, an electrical junction box used to power a light fixture had conducter wires exposed to damage. The outer insulation sheating was not connected to the strain relief.
