Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification.     At the site, employees were using a generator to supply electric to power tools with a surge protector that was not intended for use in its listing and labeling; thereby exposing employees to electrical shock and fire hazards.

29 CFR 1926.501(b)(10):     "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.    At the site, there were employees removing roofing material on a flat roof by dumping the material by hand and with a wheelbarrow into a dumpster without the use of fall protection, thereby exposing the employees to a fall hazard of 16 feet.

29 CFR 1926.501(c)(1):     When an employee is exposed to falling objects, the employer shall have each employee wear a hard hat and shall implement one of the following measures: Erect toeboards, screens, or guardrail systems to prevent objects from falling from higher levels.    At the site, there were employees gaining access onto an extension ladder while being exposed to struck-by hazards from a stack of masonry bricks and roofing material from overhead at the edge of the roof.

29 CFR 1926.502(f)(1)(i):     When mechanical equipment is not being used, the warning line shall be erected not less than 6 feet (1.8 m) from the roof edge.    At the site, employees were performing roofing work on a flat roof without the warning lines erected at least six feet from the roof's edge, thereby exposing the employees to a fall hazard at approximately 16 feet.
