29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b) Note: If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502. On or about 02/17/2012, enployees engaged in roof repair work on a two story residential building, located at 13805 Ambros Circle, Florissant, Missouri, were not protected from falling to the ground below. The height of the roof from ground to eve was more than 10 feet. The pich of the roof was approximatlely 8:12. No means of protection was used.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about 02/12/2012, employees engaged oin roof repair of a two story residential building located at 11308 Ambros Cir., Florissant, Missouri were not given training on hazards of falling and how to minimize the hazards. The height of the roof from tyhe ground to to eave was more than 10 feet. The pitch of the roof was approximatlely 8:12.
