The employer did not reasonably estimate employee exposures to contaminants including, but not limited to, anhydrous hydrogen fluoride, fluorine, carbon dioxide, and ozone when workers open the reactor doors, load/unload the reactor baskets/bins into/from the reactors, and when packaging treated bottles removed from the reactor. Detector tube readings showed concentrations a minimum of approximately 3.5 ppm up to approximately 9 ppm of hydrogen fluoride/fluorine over the reactor basket/bin that was removed from the reactor.

29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: Location: The Fluoro-Seal facility at or near 6821 Hazelwood Ave., St. Louis, MO 63134. The employer did not ensure that mandatory users of tight-fitting respirators were provided with a fit test prior to initial use of respirator and every year thereafter. Mandatory users include workers who change out alumina in towers, workers who change out the anhydrous hydrogen fluoride cylinders, and personnel who have been trained to use the SCBA respirator. FLUORO-SEAL, INC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard of 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 312978406, citation number 1, item 2, issued on 5/4/2010 and was affirmed as a final order date of 5/27/2010 with respect to a workplace located at or near 202 Cascade Drive, Allentown, PA 18109.

29 CFR 1910.132(d)(1)(i): The employer did not select the types of PPE that would protect the affected employee from the hazards identified in the hazard assessment: Location: The Fluoro-Seal facility at or near 6821 Hazelwood Ave., St. Louis, MO 63134. The employer did not select the proper personal protective equipment as follows: 1.) The Job Hazard Analysis (JHA) called out gloves for various tasks including changing out the hydrogen fluoride cylinders and changing alumina. The JHA called out gloves but it did not specify the types of gloves. 2.) The JHA called out organic vapor cartridges for use when changing out anhydrous hydrogen fluoride. Organic vapor cartridges do not provide protection from hydrogen fluoride. The employer did provide workers with the correct acid gas cartridges and not organic vapor cartridges. 3.) The respirator program and the hazard communication program called out organic vapor cartridges for use when working around the potential for fluorine or hydrogen fluoride exposure. Organic vapor cartridges are not the proper cartridges for protection from hydrogen fluoride and cartridges do not provide protection from fluorine gas.
