Fayette, OH 43521; On or about March 23, 2012, the employer did not certify that employees were trained and evaluated to safely operate a powered industrial truck. Employees are required to unload trailers of plastic totes and move pallets of cardboard totes full of plastic pellets around the facility, exposing them to struck by hazards.
