29 CFR 1910.132(a):     Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    Wellsite:  On or about March 7, 2012, employees operating a service rig during the repair of an oil/gas well did not use personal protection equipment in the following instances:    a)  Employees were not wearing flame resistant clothing, exposing the employees to the hazard of fire and/or explosion.    b)  Employees working on the well's pumping unit walking beam without a fall protection system, exposing the employees to the hazard of falling over 4 feet.

This exposed them to the hazard of being struck in the eyes by debris generated by the repair activity.
