29 CFR 1910.151(c):     Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a)  Shipping / Pallet Storage:  Employees servicing batteries for electric powered indiustrial trucks were exposed to corrosive battery accid and were not provided with immediate emergency access to an eye wash and drenching station providing at least 15 minutes of continuous flow.  Observed on or about 03/23/2012.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.165(b)(2):     The employee alarm could not be perceived above ambient noise or light levels by all employees in the affected portions of the workplace:    a)  Entire Facility:  Employees are exposed to hazards due to the employee alarm system not capable of being perceived above ambient noise or light levels by all employees in the Wheel Room, Front Office, Warehouse, and Freezer Area.  Observed on or about 03/23/2012.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.303(g)(1)(iii):     The employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment 600 Volts, nominal, or less.    a) Shipping:  Access to the electrical shutoff knife switches for the PIV chargers (4), was blocked by the PIV chargers, a 42 high wall and the PIVs being charged.      	  b)  Pallet Storage Area:  Access to the electrical shutoff knife switches for the PIV chargers (2), was blocked by the PIV chargers, as well as the PIVs being charged.     Observed on or about 03/23/2012    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
