29 CFR 1917.45(k)(3):  Defects found during inspection of cranes or derricks that created a safety hazard were not corrected by a designated person before further use of the equipment.    On or about 03/14/2012, Crane#603, Horizon lines of PR, Puerto Nuevo port area, San Juan, PR.  Employees have been exposed to several safety hazards when working inside the cranes operators cabin.   Safety findings in direction to and inside the crane described below:    1.	Loose/unwelded walking metal grating near machine house at top of ladder.  2.	Trolley rails worn in need of repairs in three different locations.  3.	Improper labeling of operators console at cabin operator station.  4.	Leg #3 personnel guard cable broken.  5.	Leg #3 rail chock missing.  6.	Leg #4 rail clamps missing.  7.	Leg #2 flipper bolt on spreader bar missing.    NOTE:	Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

29 CFR 1917.45(f)(5)(i):  Glass in the cab of a crane or derrick was not safety plate glass or equivalent and good visibility through the glass was not maintained:    On or about 03/14/2012, Crane#603, Horizon lines of PR, Puerto Nuevo port area, San Juan, PR.  Employees have been working inside the cranes operators cabin where all cabin glasses; front (1), right side (2), floor (2), left side (2), were dirty preventing the operators from having a clear view during loading/unloading operations.        Note 1:      Horizons Lines of PR was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1917.45(f)(5) which was contained in OSHA Inspection Number 313533044, Citation 1 Item 2 and was affirmed as a Final Order on March 14, 2011, with respect to a workplace located at Puerto Nuevo Port area, San Juan, PR.      NOTE 2:	 Documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(d).
