29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) An employee was potentially exposed to serious injury from a fall of about 7'4" from the top of a dumpster to the ground below. The dumpster that the employee used to access a garbage bin was the middle of three dumpsters and was located at the NW corner of Kingsley Field in Klamath Falls, OR, near the Security Forces Building under construction.

29 CFR 1926.602(c)(1)(viii): A lift truck, equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, did not have the following precautions to include: (A) Use of a safety platform firmly secured to the lifting carriage and/or forks (B) Means whereby personnel on the platform can shut off power to the truck. (a) An employee was potentially exposed to serious injury when working from a trash bin that was elevated by but not firmly secured to the forks of a forklift, nor had a means to shut off power to the truck. Located at the NW corner of Kingsley Field in Klamath Falls, OR, adjacent to the Security Forces Building under construction.
