On or about 02 March 2012, employees were observed working on a scaffold platform that was positioned on the forks of a fork-lift at the construction site located near the intersection of 18th Street and South Rangeline Road, Joplin, Missouri, that was not attached to the fork-lift, and were exposed to fall hazards resulting from the scaffold platform moving unexpectedly, becoming dislodged from the fork-lift, or tipping.

Crossbraces were used as a means of access.
