29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems.     a)  At the jobsite, on or about 3/20/12:  Employees were ingaged in roofing work at approximately 20 feet ground to eave height, on a 10:12 pitch roof, without fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)  At the jobsite, on or about 3/20/12:  Employees were engaged in roofing work at approximately 20 feet ground to eave height, on a 10:12 pitch roof, without having been trained to recognize the hazards of falls or training on the use of fall protection equipment.      Abatement certification must be submitted for this item.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall:    a)  At the jobsite, on or about 3/20/12:  An employee pushed a sheet of plywood up an extension ladder as he climbed that ladder to the second story roof.  This sheet was placed onto that roof for installation.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, that enabled each employee to recognize hazards related to ladders and stairways, and that trained each employee in the procedures to be followed to minimize those hazards:    a)  At the jobsite, on or about 3/20/12:  Employees were using extension ladders to access roof surfaces located approximately 20 feet from ground, without having been trained on use of ladders and the hazards relative to thier safe use.      Abatement certification must be submitted for this item.
