29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into through holes (including skylights) by covers: At the Jobsite - A floor hole that was 3 1/2 feet deep, 6 feet wide and 20 feet long was not covered. Opeechee Construction, was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(4)(ii), which was contained in OSHA Inspection number 112525753, Citation number 1, Item number 1, and was affirmed as a final order on May 7, 2010, with respect to a workplace located at Winthrop Street, Hallowell, Maine.
