29 CFR 1926.300(b)(1): When power operated tools are designed to accommodate guards, they were not equipped with such guards when in use. (a) On or about 03/26/2012 at Route 27 & North 4th Street, Highland Park, NJ 08904: Employees were operating a Hilti HG450 4 1/2" Angle Grinder and a Centurion 3250 Generator without the guards in place. Employees were exposed to amputation hazards when the guards manufactured with these products were not covering the moving parts. (b) On or about 04/20/2012 at Route 27 & North 4th Street, Highland Park, NJ 08904: Employees were operating a Centurion 3250 Generator without the guard over the flywheel in place. Employees using the generator were exposed to amputation hazards when the guard over the flywheel was missing.

29 CFR 1926.602(c)(1)(vii): Unauthorized personnel was permitted to ride on powered industrial trucks. A safe place to ride was not provided where riding of trucks is authorized. (a) On or about 03/26/2012 at Route 27 & North 4th Street, Highland Park, NJ 08904: Employee was exposed to a struck-by hazard while riding on the side steps of a Caterpillar 420D IT Backhoe. Riders are not permitted on the equipment without a seat and a safety restraint system in place.
