29 CFR 1926.1423(h)(2):  For erecting, climbing, and dismantling work, the employer must provide and ensure the use of fall protection equipment for employees who are on a walking/working surface with an unprotected side or edge more than 15 feet above a lower level.    Located at 309 5th Avenue, New York, NY    On or about 3/24/2012    a)  Employee was not using fall arrest system when climbing/erecting the tower crane.  Employee fell approximately 26 feet while torqueing bolts on mast of a tower crane.
