29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.    a) Northeast Bindery Inc/ NJ site/Production Area: Exit door(s) not accessible from within the Production area to provide continuous egress to the outside of the facility. Two instances include:              1. Exit door at Southern end of Production area blocked by boxes and other storage material.            2. Exit door that uses a latched wooden 2X4 with chain attachment located near Packet Collating Machine was partially blocked.      Condition observed on/or about 04/03/2012.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by moving parts of machinery:           a) Northeast Bindery, Inc. NJ site - Production Area: Machines with rotating parts and pinch points were not guarded to prevent accidental contact during operation. Machines to include, but not limited to:            1. Two Lhermite EX380 Automatic Punch Machine(s) - Rotating parts approx 12" diameter were not guarded         2. Lawson Super Duty Drill Machine - The guard to the motor had been removed and not replaced         3. Scott Ten Thousand Tabbing Machine - Rotating parts approx 2-3" in diameter were not guarded at rejector area         4. Packet Collating Machine(s) - 4 smooth pulleys approx 2" and 4" in diameter, used to                      drive 1 1/2" wide flat belts were not guarded.       Condition observed on/or about 04/03/2012.

29 CFR 1910.219(c)(2)(i): Exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, were not guarded:           a) Northeast Bindery, Inc/ NJ site - Production Area: Exposed horizontal shafts on the following machines were not guarded:                1) Scott Ten Thousand Tabbing Machine - Rotating shafts 1" to 2"  in diameter and 12" long was not guarded.                2) Lhermite EX 380 Punch Machine - Exposed shaft approx 1" diameter and 24" long was not guarded.                        Condition observed on/or about 04/03/2012.
