29 CFR 1910.1025(c)(1): Employee(s) were exposed to lead at concentrations greater than fifty micrograms per cubic meter of air averaged over an eight-hour period:           a)  On or about May 7, 2012, and at times prior, firing range employees were exposed to lead at an 8-hour time-weighted average greater than the Permissible Exposure Limit of 50 micrograms per cubic meter (ug/m3).  Sampling was conducted on May 7, 2012, on three employees while they performed cleaning, maintenance and range master duties.  Employee exposures were found to be as high as 820 ug/m3 as an 8-hour time-weighted average.

29 CFR 1910.1025(e)(1)(i): Engineering and work practice controls (including administrative controls) were not implemented to reduce and maintain employee exposure to lead in accordance with the schedule in Table I of this paragraph:         a)  On or about May 7, 2012, and at times prior, the employer failed to reduce and maintain employee exposures to lead to the lowest feasible levels through effective engineering and work practice controls.  Air sampling was conducted on May 7, 2012, on three employees while they performed cleaning, maintenance and range master duties which showed 8-hour TWA exposures to be as high as 820 ug/m3.

29 CFR 1910.1025(f)(3)(i)[A]: The employer did not select, and provide to employees, the appropriate respirators specified in paragraph (d)(3)(i)(A) of 29 CFR 1910.134:    a)  On or about May 7, 2012, and at times prior, firing range employees were exposed to lead at an 8-hour time-weighted average (TWA) greater than the Permissible Exposure Limit of 50 micrograms per cubic meter (ug/m3) because the employer did not select and provide appropriate respirators to employees.  Personal sampling conducted on May 7, 2012, showed one employee was exposed to an 8-hour TWA of 820 ug/m3 of lead while wearing a half face respirator that had an assigned protection factor of 10.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:                                                                                                                                              a)  On or about March 27, 2012, the kitchen lunch table used by employees had a surface lead contamination of 295 micrograms/square foot (ug/ft2).       b)  On or about March 27, 2012, two classroom tables used by employees had surface lead contamination of 801 ug/ft2 and 432 ug/ft2.      c)  On or about May 7, 2012, a classroom table used by employees had surface lead contamination of 359 ug/ft2.    d)  On or about May 7, 2012, the kitchen lunch table used by employees had a surface lead contamination of 283 ug/ft2.    e)  On or about May 7, 2012, the floor at the door to the trap collection room had a surface lead contamination of 1269 ug/ft2.

29 CFR 1910.1025(i)(3)(i): Employee(s) exposed to lead in excess of the permissible exposure limit (PEL), without regard to the use of respirators, were not required to shower at the end of the work shift:         a)  On or about May 7, 2012, and at times prior, firing range employees who were exposed to lead above the permissible exposure limit (PEL) did not shower at the end of the shift in order to remove lead contamination from their skin and hair.
