29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.    On March 23, 2012, employees were working to replace a valve at the base of a 4 ? inch diameter X 48 foot long deluge pipe for Tank #158 at Western Refining North Tank Farm.  Employees cut the base of the vertical pipe which severed the base support prior to and without:   1.) conducting a survey of all hazardous energy involved  2.) assigning duties and responsibilities for personnel involved  3.) determining safe shut down, de-energization, and placement of energy control devices  4.) developing written procedures that involved means and methods of control and verification steps  5.) training personnel about the hazards and procedures involved.  This condition exposed employees to potential struck-by hazards.
