29 CFR 1926.451(f)(14): Makeshift devices, such as boxes and barrels were used on top of scaffold platforms to increase the working level height of employees. The employer was using a makeshift platform consisting of concrete block and a scaffold board to increase the height of the working level of the scaffold. Employees were exposed to a fall hazard of approximately eleven (11) feet to the ground below.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a. The employer did not provide guardrails or other fall protection on the upper working level of the scaffold for employees performing masonry operations. Employees were exposed to a fall hazards in excess of ten (10) feet to the ground below. b. The employer did not ensure that the midrail was installed in the center section on the upper working level of scaffold while performing masonry operations. Employees were exposed to a fall hazards in excess of ten (10) feet to the ground below.

Employees were exposed to hazards associated with scaffold collapse.
