29 CFR 1910.23(c)(1):     29 CFR 1910.23(c)(1):  Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    a)  403 N. W.W. White Rd., San Antonio, TX,  On or about 03/06/12, and at times prior thereto, an employee was exposed to the hazards of falls from an elevation of 8 feet 9 inches to the concrete ground below while working from a Hydro Pack work platform , with a 16 inch section of top railing missing.    b)  403 N. W.W. White Rd., San Antonio, TX,  On or about 03/06/12, and at times prior thereto, an employee was exposed to the hazards of falls from an elevation of 7 feet 7 inches to the concrete ground below while working from a Pipe Machine platform, with a top rail of approximately 27 inches.     THE HANSON PIPE AND PRECAST LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1910.23(C)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315450015, CITATION NUMBER 01, ITEM NUMBER 003 AND WAS AFFIRMED AS A FINAL ORDER ON 29 JULY 2011 WITH A FINAL ABATEMENT DATE OF 15 AUGUST 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 451 WEBB RD, DUNN, NC  28334.

29 CFR 1910.24(b):     Fixed stairs were not provided for access from one structure level to another where operations necessitate regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:    a)  403 N. W.W. White Rd., San Antonio, TX,  On or about 03/06/12, and at times prior thereto, An employee accessed the Hydro Pack work platform by stepping onto the hydraulic piston outer cylinder frame, exposing them to a fall hazard to the concrete floor below.

29 CFR 1910.27(b)(1)(iii):     The minimum clear length of rungs or cleats was not 16 inches:    a)  403 N. W.W. White Rd., San Antonio, TX,  On or about 03/06/12, and at times prior thereto, an employee was exposed to the hazards of falls from an elevation of 7 7 to the concrete ground below while accessing a Pipe machine platform, from a job made ladder with rungs that were approximately 11 inches wide.

29 CFR 1910.27(c)(4):     "Clearance in back of ladder." The distance from the centerline of rungs, cleats, or steps to the nearest permanent object in back of the ladder was less than 7 inches, except that when unavoidable obstructions are encountered, minimum clearances as shown in figure D-3 shall be provided:    a)  403 N. W.W. White Rd., San Antonio, TX,  On or about 03/06/12, and at times prior thereto, an employee was exposed to the hazards of falls from an elevation of 7 7 to the concrete ground below while accessing a Pipe machine platform, from a job made ladder with the distance from the centerline of the rungs to the back of the I beam was approximately 5 inches.
