29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Located at the facility, energy control procedures to control the hazardous energy on equipment with multiple sources of energy such as electric, hydraulic, pneumatic and mechanical energy, were not developed, documented or used. This condition exposes employees to struck by and caught in hazards. Employees perform service and maintenance on equipment including, but not limited to, the following: a) Warehouse 1: 1. H-10 Hyd-Mech cutting saw (GAR 688), [electric, hydraulic]; 2. H-12 Hyd-Mech cutting saw (GAR 801), [electric, hydraulic]; 3. Landis thread cutter (GAR 988), [electric, mechanical]; 4. Peddinghaus shear (GAR 978), [electric, pneumatic]; b) Warehouse 2: 1. Kinefac MC-25 Kine-Roller (GAR 618), [electric, hydraulic]; 2. Landis thread cutter (GAR 612), [electric, mechanical]; c) Warehouse 5: 1. H-16 Hyd-Mech cutting saw (GAR 542), [electric, hydraulic]; and d) Warehouse 6: 1. SL-20 Haas lathe (GAR 769), [electric, hydraulic, pneumatic].

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Located in Warehouse 1, the unused portions of the blades on the following cutting saws were not covered exposing employees to struck-by hazards: 1. H-10 Hyd-Mech cutting saw (GAR 688) and 2. H-12 Hyd-Mech cutting saw (GAR 801). b) Located in Warehouse 6, a Lansing 18 inch lathe (GAR 613) was not equipped with a chuck guard exposing employees to caught-in hazards.
