29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from fallingor flying objects, or from electrical shock and burns, shall be protected by protective helmets. a) On or about 03/22/12, and at times prior thereto, an employee was exposed to a struck-by hazard while working below an employee who was hoisting tools and equipment overhead. The employee working at ground level was not wearing a hardhat.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed. a) On or about 03/22/12, and at times prior thereto, employees were exposed to a fall hazard of approximately 8 feet to the roof of the canopy. While accessing the canopy, from an adjacent roof, the employees using an step ladder in the closed position. The step ladder was designed to be used with the legs open and spreader bar locked.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder shall not be used as a step. a) On or about 03/22/12, and at times prior thereto, an employee was exposed to a fall hazard of approximately 8 feet to the roof of the canopy. While accessing the canopy, from an adjacent roof, the employees used the top and top step of the step ladder, while the ladder was in the closed position.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. a) On or about 03/22/12, and at times prior thereto, employees were exposed to a fall hazard while using step ladders. The employees utilized two step ladders at the site. Both step ladders were used incorrectly which demonstrates a need for additional employee training.
