29 CFR 1910.110(f)(2)(i):     Containers in storage were not located so as to minimize exposure to excessive temperature rise, physical damage, or tampering by unauthorized persons.    a)  At the camp host pads the DOT 4BH240 propane tanks waiting to be used when the camp hosts arrived were not located so as to minimize exposure to physical damage.

29 CFR 1910.253(b)(4)(iii):     Oxygen cylinders in storage were not separated from fuel-gas cylinders  a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.    a)   Oxygen cylinders and fuel-gas cylinders were stored together on a cart but were not in use.

29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:    a)  In the Molalla Road Maintenance Shop rest room the pressure relief valve drain line on a water heater terminated at 15 inches from the floor and 26 inches from the toilet seat.  Per the Oregon Plumbing Specialty Code the discharge location of the drain line on a water heater shall be not less than 6 inches from the floor level.    Feasible and useful abatement methods for reducing this hazard include, but not limited to, extending the drain line to not less than 6 inches from the floor level, as required in the 2011 Oregon Plumbing Specialty Code 608.5, as enforced by the Clackamas County Building Codes Division.

29 CFR 1910.303(g)(2)(i):  Live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a)  In the steam cleaner shed the electrical conductors and contacts on the bottom of the lamp holder for the heat lamp were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures as the lamp holder was not affixed to the junction box.     The employer was cited for a violation of the same standard and/or same hazard on 03/09/2011 Inspection #307507285, date 03/21/2011 of Final Order/settlement, and means of abatement.
