29 CFR 1910.304(b)(3)(i): All 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms or on rooftops did not have ground-fault circuit-interrupter protection for personnel. a) In the Mountain View rest room, on the Mens' side, the ground-fault circuit-interrupter was not operative and thus the employees were not protected.

29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: a) In the Mountain View rest room building, in the janitorial room between the rest rooms, the pressure relief valve drain line terminated at 38 inches from the floor. Per the Oregon Plumbing Specialty Code the discharge location of the drain line shall be not less than 6 inches from the floor level. Feasible and useful abatement methods for reducing this hazard include, but not limited to, extending the drain line to not less than 6 inches from the floor level, as required in the 2011 Oregon Plumbing Specialty Code 608.5, as enforced by the Clackamas County Building Codes Division.
