29 CFR 1926,453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: At the north side of a strip mall building at 12151 K Westheimer, two employees were working in an Altec truck-mounted aerial lift basket, license plate number 12Y2T5, without body belts or lanyards while installing a sign on the building. This condition exposed the employees to potential fall hazards in excess of ten feet. BILLY NGUYEN DBA UNITED SIGNS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312921810, CITATION NUMBER 1, ITEM NUMBER 2, AND WAS AFFIRMED AS A FINAL ORDER ON MAY 12, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 8817 HIGHWAY 6, STE 400, MISSOURI CITY TEXAS 77459.
