29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a) Overcashier and Horst Heating and Air Conditioning, Inc., Holland, Ohio: On or about March 28, 2012, at the address of 1494 Spring Meadows Drive, Holland, OH, the employer did not ensure that each employee working on a commercial structure with a flat roof were protected from fall hazards. Employees were preparing HVAC units for removal and were exposed to falls of approximately 15 feet to the ground below, which could result in permanent disability or death.
