29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment was performed through a written certification:     (a) Re-Steel Supply Company  Specialty Metals Division, dba Re-Steel Supply Company @ 8680 Ulster St., Commerce City, CO 80022: On and before 3/29/12, the employer did not maintain a written certification of a workplace hazard assessment. Employees are exposed to hazards such as, but not limited to, flying debris, ultraviolet radiation, metal fume from plasma cutting, and noise. The employer requires employees to wear personal protective equipment such as, but not limited to, eye protection, welding helmets, hearing protection, and respiratory protection.
