29 CFR 1926.502(d)(6)(ii): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself. Instance A.) On or about March 29, 2012 a non-locking snap hook from a 6 foot shock absorbing lanyard was attached to a retractable device snap hook and used for fall protection. Instance B.) On or about March 29, 2012 a non-locking snap hook from a 3 foot shock pack lanyard was attached to a retractable devices snap hook and then the other non-locking snap hook was attached to a non-locking anchor snap hook and used for fall protection. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c)

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system. On or about March 29, 2012 employees were on a roof performing roofing work without fall protection exposing the employees to a fall hazard of greater than 25 feet. All American Commercial Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29CFR 1926.501(b)(10) which was contained in OSHA inspection number 311373096, citation number 1, item number 1, and was affirmed as a final order on 05/15/2008, with respect to a workplace located at 6624 S. Cicero Ave., Bedford Park, Illinois. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
