29 CFR 1926.451(c)(2)(v):     Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    On or about 03/27/12 during the installation of a fall protection system and other construction activities, the employer did not ensure that an all-terrain forklift that was supporting a scaffold platform and a wooden pallet was secured to the forks to prevent movement.

29 CFR 1926.452(k)(2):     All ladders used to support ladder jack scaffolds did not meet the requirements of subpart X of thispart -- Stairways and Ladders, except that job-made ladders shall not be used to support ladder jack scaffolds:    On or about 03/27/12 during the installation of a fall protection system and other construction activities, the employer did not ensure that a portable stepladder that was used as a scaffold plank support met the requirements of subpart x.  The stepladder was in the closed position and was leaning against a wall as it supported a scaffold plank.

COMMERICAL ROOFING INC.
