29 CFR 1910.269(d)(6)(iii): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or about March 29, 2012, and at times prior, employees were exposed to a release of hazardous energy from failing to isolate all energy sources prior to starting work associated with the Air Handler Demolition project at the Pueblo Comanche Power Station.This resulted in a contract employee receiving 3rd degree burns when hot water/steam was released during a line breaking job.
