29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Machine Shop, Ikeda Radial Drill, Model: 1300 S/N 6649: On or about 03/21/12 and at times prior thereto employees were exposed to an amputation hazard when drilling holes in an aircraft engine component. b) Machine Shop, Kearney and Trecker Milling Machine, Model: 205 S-12, S/N 53-1013-04: On or about 3/21/12 and at times prior thereto employees were exposed to an amputation hazard when milling aircraft engine components. c) Machine Shop, Monarch Lath Model: EE, S/N 45837: On or about 3/21/12 and at times prior thereto employees were exposed to an amputation hazard when turning aircraft engine parts.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Machine Shop, Valley Bench Grinder, Model: GFE 11773: On or about 03/21/12 and at times prior thereto employee(s) were exposed to a struck by hazard when deburring parts from a tongue guard being approximately 3/4 inch from the abrasive wheel.
