This condition exposed the employee to a struck-by hazard.

29 CFR 1926.403(b)(1):The employer did not ensure that electrical equipment was free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: a) On or about March 21, 2012 and at times prior therto, at this job site, a flexible electrical extension power cord without a grounding pin on the electrical plug were used to power tools such as but not limited to an electrical hammer breaker. This condition exposed the employee(s) to an electrical shock.
