29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: Location: In the Laminating Area of the Georgia Pacific manufacturing area in Cuba, MO The employer did not ensure that the Laminator #1 machine (the north machine) was properly guarded as follows: 1.) The access panel to the south saw blade did not require a tool to access the blade. A worker's finger was amputated after opening the access panel to clear a jam in the south saw. 2.) The gear/chains were not protected to prevent employee contact. 3.) The top of the machine was not guarded to prevent workers from accessing the machine to oil the bearings while the machine is running. 4.) The stile positioning rams were not properly guarded to prevent workers from accessing the area when the rams were activated. 5.) The bottom of the machine was not guarded to prevent workers from
