29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system: a) South side roof of Buildling "B", on or about 3/30/12: Two employees engaged in roof work on a low sloped roof were not protected from falling 48 feet 6 inches to grade. b) South side roof of Building "B", on or about 4/2/12: An employee engaged in roofing work on a low sloped roof was not protected from falling 48 feet 6 inches to grade.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. a) On the roof of Building "B", on or about 4/2/12: The anchorage point being used by the owner of the company to secure his fall protection was not installed properly. Only one side of the bracket was secured to the roof deck, the manufacturer requires both sides to be secured.
