29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Wlker Rd. & LA. 23, Belle Chasse, LA.; The employer failed to ensure that the excavation employees were working in had a sufficient ladder for safe access and egress of the excavation. The excavation was 7 feet deep and the ladder being used was a 6 foot Werner step ladder.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that: a) Walker Rd. & LA. 23, Belle Chasse, LA.; Employees were observed installing manholes/catch basins in a 7 feet deep excavation with vertical sides and no adequate protective system in place. This condition exposed employees to hazards associated with a trench collapse/cave in.
