29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a.) Back Production Area - Live electrical parts were not guarded, as required on, the back end of the Stokes Molding Machine #4, exposing employees to electrical shock hazards, on or about 3/20/2012. Abatement verification is not required.

29 CFR 1910.303(g)(1)(iii): At least one entrance of sufficient area shall be provided to give access to the working space about electric equipment. a. Front Manufacturing Area - The only point of access to a series of electrical panel boxes in the front manufacturing area was used for storage which restricted the access width to only 13 inches, limiting quick access to the panels in an emergency, on or about 03/20/2012. No abatement notification required.
