Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated: At 3725 E. Texas St., Bossier City, LA 71111; the employer did not ensure that electrical cords were visually inspected for external defects before each use: a) In the welding shop; on or about March 22, 2012, there was insulation damage (with exposed wiring) on the electrical cord of a Craftsman 7 industrial angle grinder (Model 900.277120, S/N 049097), exposing employees to the hazard of electrical shock. b) In the welding shop; on or about March 22, 2012, the electrical plug pins and plug outer jacket on the electrical cord of a Craftsman 7 industrial angle grinder (Model 900.277120, S/N 049097) were damaged and unsuitable for safe use, exposing employees to the hazard of electrical shock.
