29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    At 3725 E. Texas St., Bossier City, LA 71111; the employer did not ensure that each saw was properly guarded.  This condition was observed on or about March 22, 2012, where the portion of the saw blade above the table on a Ridgid table saw (Model TS3660; S/N P080411848) was not enclosed by an adjusting hood, exposing the operator to amputation/laceration hazards.

Texas St., Bossier City, LA 71111; the employer did not ensure that each portable grinder was properly guarded.  This condition was observed on or about March 22, 2012, where a Craftsman 7-inch industrial angle grinder (Model 900.277120, S/N 049097) did not have an abrasive wheel guard installed, exposing employees to amputation, laceration and/or struck-by hazards.

This condition was observed on or about March 22, 2012, in the welding shop area, where there was a circuit breaker opening in a Square D electrical panelboard, exposing employees to electric shock hazards.

This condition was determined on or about March 22, 2012 where the following case was not entered on the OSHA 300 Log for year 2011 within seven days after receiving information:    Employee (Case No.1) fell off a ladder while attempting to install plywood to the ceiling of a shelter and injured right knee that resulted in job transfer or restrictions of 55 days.

This condition was observed on or about March 22, 2012 in the welding shop area and throughout the site where portable fire extinguishers had not received an annual maintenance inspection since year 2004 and were not visually inspected by the employer at least monthly.
