29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: a.) Cut-off Saw Operator-An employee was exposed to continuous noise at 88.1% of the allowable eight (8) hour time weighted sound level of 80 dBA or an equivalent sound level of approximately 89.1 dBA and an audiograms were not provided. The sampling was performed for 97 minutes on 3-22-12. Zero exposure was assumed for the unsampled period of time. b.) Bandsaw Operator A-An employee was exposed to continuous noise at 174.3% of the allowable eight (8) hour time weighted sound level of 80 dBA or an equivalent sound level of approximately 93 dBA and audiograms were not provided. The sampling was performed for 144 minutes on 3-22-12. Zero exposure was assumed for the unsampled period of time. c.) Bandsaw Operator B-An employee was exposed to continuous noise at 81.6% of the allowable eight (8) hour time weighted sound level of 80 dBA or an equivalent sound level of approximately 88.5 dBA and audiograms were not provided. The sampling was performed for 140 minutes on 3-22-12. Zero exposure was assumed for the unsampled period of time. ABATEMENT CERTIFICATION REQUIRED
