29 CFR 1926.451(a)(1): Except as provided in paragraphs (a)(2), (a)(3), (a)(4), (a)(5) and (g) of this section, each scaffold and scaffold component shall be capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: a) On March 27, 2012 at the job site located at 3171 W. Broadway in Sedalia, MO, employees were exposed to fall from height hazards in that the elevated work platform on which they were working was approximately 16' wide and the rough terrain forklift supporting that platform had an approximate wheelbase of 8'2", exceeding the maximum allowance of the platform being 10" wider than the wheelbase. Abatement certification is not required for this violation.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attachedto the fork and the fork-lift is not moved horizontally while the platform is occupied: a) On March 27, 2012 at the job site located at 3175 Broadway in Sedalia, MO, employees were exposed to fall from height hazards in that the supported scaffold platform on which they were working was moved horizontally by the rough terrain forklift which was supporting the scaffold. Abatement certification is not required for this violation.
